This section of the Medical Home Portal provides primary care clinicians, care coordinators, and other health care professionals with information, tools, and resources to enable ongoing improvement in:
  • Care and outcomes for patients
  • Patient- and family-centered care

The Patient-Centered Medical Home provides an overview of the components of a medical home and the value it offers providers and families.
Coding & Billing helps clinicians understand available codes to support better billing and compensation, particularly for services provided to children and youth with special health care needs (CYSHCN).
About Children & Youth with Special Health Care Needs (CYSHCN) looks at the overriding care issues facing CYSHCN.
Care Coordination discusses how to coordinate care for those who could benefit most from it, code for these related services, and find training materials for improving the “medical homeness” of your practice.
Common Issues for CYSHCN provides clinical information about health issues for CYSHCN.
School Accommodations: IEPs & 504s shows how educators, schools, and school systems can help your patients and how you can help ensure schools are providing appropriate services for them. Feeding & Nutrition addresses some of the causes of undernourishment, which is common in many children with chronic conditions and genetic syndromes. It provides evaluation and assessment tips for clinicians, detailed information about formulas, and information about calcium and vitamin D recommendations.
Guidelines & Algorithms has care processes for evaluation, screening, and management care. The Clinical Guidelines for Care of Children with Medical Complexity contain published guidelines for diagnosis and management of chronic conditions in children; many of the conditions have corresponding modules that can be found at Diagnoses & Conditions.
Medical Technology refers to devices and modifications that replace or support physiologic function. Each technology reviewed in this section includes information about medical indications for use, examples of medical orders to prescribe the technology, and tips for effectively communicating with home health care. It also addresses many common clinical questions and concerns.
Screening & Prevention offers guidelines and resources for screening for developmental, sensory, nutritional, and other health-related problems to allow early intervention for or prevention of chronic conditions and their complications.
